All students trying out for a sports team must have the Archdiocese of Miami Athletic Consent & FHSAA Physical Evaluation forms completed each school year. Pages 3 and 4 of the Physical Evaluation must be completed and signed/dated by the child’s doctor after June 1st for each new school year. Students need to turn in their complete sports forms to the Athletic Directors before trying out or practicing for any SCS sports team.
